Any size as long as they don't sag or they get major support from a bra. It's not even indifference, it's that flat has a different appeal than small buds, which are different from small-medium spherical ranges, and there's a different appeal entirely from the extremely oversized/implants. And this is regardless of gender or sex, so long as there's not significant chest hair.

Depends on age. If the female is below the single digits, obviously a flat chest is good. But above 15, the female should be well-endowed.

You speak my mind. In addition to the emotional satisfaction, these pointy buds are more sensitive compared with larger breasts, which means more motion, feeling, and fun.
